This section contains various forms that may be required and documents for reference. Forms can be downloaded and printed off at your convenience. Completed forms should be posted to the Schools Office at Thomond Park. You should retain a copy of the completed form for your own records.

The forms are available as PDF. In order to open/print these forms you will need to have Adobe Acrobat Reader installed on your computer. If you do not have this installed you may download it freely from the Adobe website. A link to this is provided to the left.

NOTES ON THE COMPLETION OF THE AFFILIATION FORM for 2006/7

You are asked to carefully complete the affiliation form as the form will act as a basis for

(a) Listing affiliated schools in the Munster Branch handbook for next season.

(b) The compilation of a Mail List which enables the Branch and the Schools Committee circulate information and occasional updates.

(c) Corresponding with schools with regard to Cup Competitions and European Rugby and Celtic Cup tickets and other developments. It is the school club secretary only who will be contacted in this regard. Even if you do not wish your home number to be included in the branch booklet, it is advisable that you forward this office your home number in the event of urgent information. This was important this year as demand for ERC tickets could only be processed by phone at one point of time. Only affiliated schools are forwarded booking forms for international tickets each September. It is up to each school to complete and return such forms. For ERC tickets, only schools who order tickets and who forward blank cheques to the Schools office in advance will have tickets allocated to them.

(d) Enabling schools have a direct information and/or contact line with the Munster Schools Committee.

The form asks for an email address for the school. You are asked to include a personal email address for the individual regularly dealing with rugby matters. If such a person doesn’t have an email address, please offer the email address of another person in the school who has and who will relay the information contained therein. It is no good submitting an email address which doesn’t function. Email addresses are becoming more important in terms of circulating information quickly.

The Munster Schools Office in Thomond Park will normally make contact with schools through email. You are advised to check on a regular basis. The Munster Schools email address is [email protected]

Submission of this form is an essential and prior requirement before inclusion in any competition for next year and inclusion in the branch handbook.

As far as insurance is concerned. The IRFU pays the insurance premium for affiliated schools. The details of the insurance scheme are included in the Munster Branch Handbook which is issued each year. You are advised to consult this. You may wish to supplement cover for players. This is for the individual and/or the school to arrange.
